Here is what U.S. physicians and medical practices should understand before the final rule.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Medicare Physician Fee Schedule (PFS) is entering another important period of change.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">On July 14, 2026, the Centers for Medicare &amp; Medicaid Services (CMS) released its <strong>Calendar Year (CY) 2027 Medicare Physician Fee Schedule proposed rule<\/strong>, outlining proposed changes to Medicare Part B payment policies and related physician-practice requirements. The proposal covers physician reimbursement, evaluation and management (E\/M) services, practice expense methodology, remote physiologic and remote therapeutic monitoring, quality reporting, interoperability, accountable care, primary care payment, and other areas affecting medical practices.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The public comment period closed on <strong>September 14, 2026<\/strong>. As of October 1, 2026, the 2027 PFS provisions discussed in this article remain <strong>proposed<\/strong>, not final. CMS is expected to issue the final rule before the major provisions scheduled to take effect on January 1, 2027. The final policy could differ from the proposals described below.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For physicians, practice administrators, billers, coders, revenue-cycle teams, and healthcare organizations, the significance goes beyond the headline conversion factor.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The proposed rule could change how certain services are valued, documented, coded, billed, and monitored. That makes 2026 an important preparation period for practices that depend substantially on Medicare reimbursement.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">What Is the Medicare Physician Fee Schedule?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong>Medicare Physician Fee Schedule (MPFS)<\/strong> is the system CMS uses to establish payment rates for many services provided by physicians and other eligible healthcare professionals to Medicare beneficiaries.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Under the PFS, services are generally valued using <strong>Relative Value Units (RVUs)<\/strong>. CMS considers several components, including:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Work RVUs<\/strong> \u2014 physician work and clinical decision-making involved in a service<\/li>\n\n\n\n<li><strong>Practice Expense (PE) RVUs<\/strong> \u2014 resources required to furnish the service<\/li>\n\n\n\n<li><strong>Malpractice RVUs<\/strong> \u2014 professional liability costs<\/li>\n\n\n\n<li><strong>Geographic Practice Cost Indices (GPCIs)<\/strong> \u2014 geographic adjustments<\/li>\n\n\n\n<li><strong>Conversion factor<\/strong> \u2014 the dollar conversion applied to the applicable RVUs<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The basic payment calculation can therefore be simplified as:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Medicare payment \u2248 [(Work RVU + Practice Expense RVU + Malpractice RVU) \u00d7 Geographic adjustments] \u00d7 Conversion Factor<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Actual Medicare payment calculations are more complex and depend on the service, setting, locality, modifiers, multiple-procedure rules, and other applicable policies.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">CMS explains that PFS payments have been based on relative resources and conversion factors since the physician fee schedule was established in 1992.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For medical practices, this means that a change in the conversion factor is only one part of the reimbursement equation. Changes to RVUs, coding policies, practice-expense methodology, and service-specific valuation can also influence reimbursement.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">What Is Changing in the 2027 Medicare Physician Fee Schedule Proposal?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Several provisions deserve particular attention from physicians and medical practice leaders.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">1. Proposed 2027 Medicare Conversion Factors<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">One of the most immediately relevant issues is the proposed Medicare conversion factor.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Beginning in 2026, Medicare has two statutory conversion factors:<\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li>One for qualifying Alternative Payment Model (APM) participants<\/li>\n\n\n\n<li>One for physicians and practitioners who are not qualifying APM participants<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">For 2027, CMS proposes:<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><tbody><tr><th>Category<\/th><th>Proposed 2027 Conversion Factor<\/th><th>Approximate Change From 2026<\/th><\/tr><tr><td>Qualifying APM participants<\/td><td><strong>$33.17<\/strong><\/td><td><strong>-1.19%<\/strong><\/td><\/tr><tr><td>Non-qualifying APM participants<\/td><td><strong>$32.84<\/strong><\/td><td><strong>-1.68%<\/strong><\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">CMS says the proposed changes reflect statutory updates, including the expiration of the temporary 2.5% increase that applied to the 2026 PFS conversion factor, along with other adjustments.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">These figures are <strong>proposed<\/strong>, not final.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">More importantly, physicians should not interpret the conversion-factor percentage as a uniform reduction across every service. Individual service payments can move differently because CMS is simultaneously proposing changes to work RVUs, practice-expense methodology, coding, and other payment policies.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Why this matters to medical practices<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Practices should model the potential impact at the <strong>service and specialty level<\/strong>, rather than applying the conversion-factor percentage across total annual Medicare revenue.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For example, a practice should examine:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Medicare allowed charges by CPT\/HCPCS code<\/li>\n\n\n\n<li>Medicare volume by service<\/li>\n\n\n\n<li>Work RVUs<\/li>\n\n\n\n<li>Practice-expense RVUs<\/li>\n\n\n\n<li>Modifier utilization<\/li>\n\n\n\n<li>E\/M distribution<\/li>\n\n\n\n<li>Global surgical services<\/li>\n\n\n\n<li>Remote monitoring revenue<\/li>\n\n\n\n<li>APM participation<\/li>\n\n\n\n<li>Medicare payer mix<\/li>\n\n\n\n<li>Denial and underpayment patterns<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">This is where revenue-cycle analytics can become especially valuable.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">2. Same-Day E\/M Services and Global Procedures<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">One of the most consequential proposals for many physician practices concerns <strong>evaluation and management (E\/M) services performed on the same day as procedures with global periods<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">CMS proposes that when a separately identifiable office\/outpatient E\/M service is furnished by the same physician\u2014or a physician in the same practice\u2014on the same day as a procedure with a 0-, 10-, or 90-day global period:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The more expensive service would be paid at 100%.<\/li>\n\n\n\n<li>Other applicable surgical procedure(s) or E\/M service(s) on that day would be paid at <strong>50%<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">This is a <strong>proposed policy<\/strong> and could change before the final rule.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The proposal is particularly important for specialties and practices that frequently report both procedural services and separately identifiable E\/M services.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">What practices should review<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Before the final rule, organizations can analyze historical claims involving:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Modifier 25<\/li>\n\n\n\n<li>Office\/outpatient E\/M codes<\/li>\n\n\n\n<li>0-day global procedures<\/li>\n\n\n\n<li>10-day global procedures<\/li>\n\n\n\n<li>90-day global procedures<\/li>\n\n\n\n<li>Same-day E\/M and procedure combinations<\/li>\n\n\n\n<li>Medicare allowed amounts<\/li>\n\n\n\n<li>Denials and payment adjustments<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">A historical claims analysis can help practices estimate which service combinations could be affected if the proposed policy is finalized.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">3. Proposed Changes to G2211<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">CMS is also proposing changes to the <strong>office\/outpatient E\/M visit complexity add-on code G2211<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">G2211 was implemented for payment beginning in 2024\/2025 policy implementation and is intended to recognize the additional complexity associated with certain longitudinal patient relationships.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For 2027, CMS proposes converting G2211 from a separate add-on code into a modifier structure.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Under the proposal:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>One proposed modifier would increase the associated E\/M payment by <strong>16%<\/strong>.<\/li>\n\n\n\n<li>A second proposed modifier would be available to certain practitioners participating in specified accountable care models and would increase the associated E\/M payment by <strong>32%<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The proposed ACO-related modifier is designed to recognize additional resources associated with longitudinal care, total-cost-of-care accountability, and quality reporting.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This could have implications for:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Primary care<\/li>\n\n\n\n<li>Internal medicine<\/li>\n\n\n\n<li>Family medicine<\/li>\n\n\n\n<li>Chronic disease management<\/li>\n\n\n\n<li>Accountable care organizations<\/li>\n\n\n\n<li>Medicare Shared Savings Program participants<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Practices should therefore monitor the final rule closely before changing production billing workflows.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">4. Remote Patient Monitoring Could See Significant Changes<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Remote monitoring remains another important area.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">CMS currently pays for <strong>Remote Physiologic Monitoring (RPM)<\/strong> and <strong>Remote Therapeutic Monitoring (RTM)<\/strong> services under the PFS.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For 2027, CMS proposes several changes, including:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Requiring RTM services to be furnished only to established patients<\/li>\n\n\n\n<li>Requiring a separately reportable initiating visit associated with the start of RPM or RTM<\/li>\n\n\n\n<li>Limiting payment to RPM\/RTM services performed by clinical staff employed by the practice rather than contractors<\/li>\n\n\n\n<li>Revaluing certain services based on updated assumptions regarding device costs<\/li>\n\n\n\n<li>Considering whether RPM and RTM codes should be bundled into four new HCPCS G-codes<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">CMS says the proposals respond in part to recommendations from recent Office of Inspector General reports concerning the current coding structure.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For practices that have built programs around RPM or RTM, the issue is therefore not simply coding.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It can affect the economics and operational structure of the entire program.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Practices should evaluate<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Before 2027, organizations using RPM or RTM should examine:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Patient eligibility<\/li>\n\n\n\n<li>Established-patient status<\/li>\n\n\n\n<li>Device costs<\/li>\n\n\n\n<li>Clinical staff utilization<\/li>\n\n\n\n<li>Contractor arrangements<\/li>\n\n\n\n<li>Enrollment rates<\/li>\n\n\n\n<li>Monthly billing volume<\/li>\n\n\n\n<li>Claim acceptance rates<\/li>\n\n\n\n<li>Denials<\/li>\n\n\n\n<li>Documentation completeness<\/li>\n\n\n\n<li>Revenue per enrolled patient<\/li>\n\n\n\n<li>Cost per monitored patient<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">This type of analysis can help organizations understand how potential policy changes could affect program sustainability.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">5. CMS Is Proposing Changes to Practice-Expense Methodology<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Another technically important part of the proposal concerns <strong>Practice Expense (PE)<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Practice expense is a major component of physician payment because providing healthcare requires much more than physician labor.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Medical practices must pay for:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Clinical staff<\/li>\n\n\n\n<li>Administrative staff<\/li>\n\n\n\n<li>Facilities<\/li>\n\n\n\n<li>Equipment<\/li>\n\n\n\n<li>Supplies<\/li>\n\n\n\n<li>Technology<\/li>\n\n\n\n<li>EHR systems<\/li>\n\n\n\n<li>Billing infrastructure<\/li>\n\n\n\n<li>Compliance<\/li>\n\n\n\n<li>Malpractice coverage<\/li>\n\n\n\n<li>Other operational resources<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">CMS says it is pursuing a multi-year effort to reduce reliance on older specialty-specific practice-expense-per-hour data and move toward more objective, routinely updated and auditable cost information.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The proposed methodology would phase out part of the existing approach and introduce a <strong>practice-expense stabilizer<\/strong> intended to reduce short-term volatility.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Why this matters<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Changes in practice-expense methodology can redistribute payment across services and specialties.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That means a practice should not evaluate the proposed 2027 PFS exclusively through the headline conversion-factor change.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A more sophisticated analysis looks at:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>CPT\/HCPCS volume \u2192 RVU changes \u2192 payment changes \u2192 specialty impact \u2192 Medicare revenue impact<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is particularly relevant for organizations with large multispecialty operations.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">6. MIPS and MIPS Value Pathways Are Moving Toward Another Transition<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The Quality Payment Program (QPP) remains an important part of Medicare physician participation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The 2027 proposal includes changes involving:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Merit-based Incentive Payment System (MIPS)<\/li>\n\n\n\n<li>MIPS Value Pathways (MVPs)<\/li>\n\n\n\n<li>Advanced Alternative Payment Models (APMs)<\/li>\n\n\n\n<li>Quality reporting<\/li>\n\n\n\n<li>Promoting interoperability<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">CMS is proposing to move away from traditional MIPS reporting and toward a broader MVP-based structure beginning later in the transition period.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The eCQI Resource Center reports that CMS proposed sunsetting the traditional MIPS reporting option after the <strong>2028 performance year<\/strong>, while introducing three new MVPs for the 2027 performance year related to diabetes, hypertension, and hospitalists and modifying previously finalized MVPs.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The proposed changes therefore matter particularly to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Small practices<\/li>\n\n\n\n<li>Independent physicians<\/li>\n\n\n\n<li>Specialty practices<\/li>\n\n\n\n<li>ACO participants<\/li>\n\n\n\n<li>Clinicians participating in MIPS<\/li>\n\n\n\n<li>Practices building quality-reporting infrastructure<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">7. Interoperability and Electronic Prior Authorization Remain Connected to Physician Workflow<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Although the 2027 PFS proposal covers many payment policies, interoperability remains a major operational issue for physician practices.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">CMS has separately been advancing electronic prior authorization and FHIR-based interoperability.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In 2026, CMS announced an <strong>Electronic Prior Authorization Acceleration initiative<\/strong>, involving healthcare organizations, EHR developers, physician practices, networks, digital-health developers, and major payers.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">CMS said the initiative is intended to reduce dependence on manual processes such as fax and payer portals, improve authorization-status visibility, and address workflow and technical barriers ahead of 2027 requirements.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is significant because the administrative burden associated with prior authorization remains substantial.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The AMA&#8217;s 2025 physician survey found that physicians reported an average of <strong>40 prior authorization requests per week<\/strong>, with prior authorization consuming an average of <strong>13 hours of physician and staff time each week<\/strong>. The survey also found that 94% of surveyed physicians said prior authorization contributes to burnout.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">CMS has also proposed expanding FHIR-based standards for prior-authorization-related transactions and attachments in a separate 2026 rulemaking effort.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For medical practices, this means interoperability is increasingly becoming an operational and revenue-cycle issue\u2014not simply an IT issue.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">8. AI Is Becoming Part of Physician Practice Operations<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Artificial intelligence is another major development affecting physicians and medical practices.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">An American Medical Association survey published in March 2026 found that <strong>81% of physicians surveyed reported using AI professionally<\/strong>, more than double the rate reported when the AMA first surveyed physicians on health AI in 2023.
